1) CUI to show Customize dialog
2) Switch to Toolbars section
3) Drag-n-drop required buttons straight to the Toolbars section.
New Toolbar1 will appear in the list, containing buttons you dragged out.
4) Rename Toolbar1 as you need using Rename button, use Delete to remove toolbar if you don't need it. Use Options... button for editing buttons themselves - you can pick any icon and set any command you want for buttons.
